OBJECTIVE: To analyze strategies developed in primary health care that contribute to osteoporosis prevention and the reduction of fracture risk in adults and the elderly. METHOD: An integrative literature review was conducted using the PubMed/MEDLINE database, covering publications from 2021 to 2026. After applying selection criteria, seven studies were evaluated; four were excluded for not meeting the scope, resulting in a final sample of three articles. RESULTS: The main strategies identified were the early recognition of risk factors, bone health screening, timely diagnosis and treatment, monitoring of treatment adherence, and preventive interventions to reduce the occurrence and recurrence of fractures. CONCLUSION: Primary health care plays a fundamental role in preventing osteoporosis and fractures, particularly through early risk identification, continuous monitoring, and the implementation of individualized preventive measures.
