Objectives: To evaluate the association between overweight and frailty, as well as its specific domains, in older adults with chronic kidney disease (CKD). Methods: A cross-sectional study was conducted with 65 older adults attending a nephrology outpatient clinic. Frailty was assessed using the Edmonton Frail Scale (EFS), and body composition was measured by anthropometry and bioelectrical impedance analysis. Laboratory data were obtained from medical records. Associations between BMI, frailty, and its domains were adjusted for age and sex. Results: The mean age was 69.8 years, 55.4% were men, and mean BMI was 32.6 kg/m². Frailty was identified in 58.5% of participants. Overweight individuals had lower muscle mass and higher fat mass (p < 0.01), along with greater medication use and lower urinary continence (p < 0.05). Conclusion: Overweight was associated with poorer body composition and higher frailty, highlighting its negative impact on functional health in older adults with CKD.
